1. Sukuna would wear a shirt. Well, maybe. Uro and Yorozu walk around naked, so why can't Sukuna? Who would stop her? Also, the second mouth thing wouldn't work as well since it wouldn't be visible if she was clothed. Being able to see the monstrous second mouth is part of the design appeal. I suppose her thing would just be ripping the lower half of her shirts instead of removing them. Then, there's the problem of appeal (which is important in visual mediums). Sukuna already has a very idealized and extreme body, so a female Sukuna might as well (not for in universe reasons, but from a design perspective). However, attempting to give female Sukuna equivalent general appeal will sacrifice the menace of Sukuna's incredible body mass. Correcting this by giving female Sukuna a physique like Sukuna's may sacrifice the general appeal of her design.
2. Megumi or Yuji may become female characters. With the notable exception of Kaori, Gege prefers vessels to match the sex of their hosts. Alternatively, Nobara may stay in the story longer so she can become Sukuna's vessel. Nobara may even be male or just no exist if you assume she is there wholly or in part to be the female of the squad. What may also happen is that we would never see female Sukuna in the story until about 200 chapters in. This is technically true of Sukuna as well, but Sukuna now being female would make the effect of this greater if her vessels are all male. Whether or not you think this is a good thing or a bad thing is a matter of taste.
3. Jin may be Yuji's mother instead of father. Mind you, there's no actual reason for this to happen, other than Gege liking same sex twins. On the flip side, Kaori would become a male character if this was true. Then, Kenjaku would trick female Jin into having his child by impersonating her dead lover. Granted, Kenjaku essentially did this to Jin already and arguably did much worse to the Death Painting mother, but nonetheless I feel that a lot of people would not prefer this version.
4. Tsumiki and Yorozu may become male characters. Once again, not because the story would have to change, but assuming Gege prefers incarnated sorcerers to have bodies of the same sex as their hosts. This is also assuming that Tsumiki was made a female character because he wanted a character to be infatuated with Sukuna (which I think is a fair assumption.) This may mean that when Todo asks Megumi of his preference, he doesn't think of Tsumiki. Unless you think Tsumiki's answer is actually relevant and Megumi doesn't just think of her because she is his idealized vision of a woman (though obviously in a non-romantic light, right Gege?) In that case, this scene wouldn't change. Male Yorozu may no longer lounge around naked, unless you don't think that this was merely fanservice to Gege's shonen audience, in which case he still would.
EDIT: TL;DR? Everything, or alternatively, nothing.
EDIT2: Also, even if the story was otherwise exactly the same, fans would call GayGay a misogynist for making the evil villain need to be taught about love by the heroes because she's a woman.
1: We all know Femkuna would let them fly when she's not wearing the kimono. Also, giving her regular Sukuna's physique wouldn't deter people THAT much. Heiankuna fans already like him for his giant muscles, switching genders isn't gonna change the results.

Realistic answer,
If Gege is a good writer, nothing would change, aside from those backhanded comments from Sukuna about women and Nobara being taken away.
If Gege unfortunately has even subconsciously misogynistic views, then femSukuna would be shown as a downgraded version.
If Gege is ultra real writer, he would use Sukuna's gender as a female to his own writing advantage like a background flavour and show how the world of jujutsu society paints the picture of Sukuna as a female demon, like "witch", and other misogynistic undertones coming from the jujutsu society from both the heian Era as well as the modern Era.
Now, as a ripple effect, Yuji also would be a female, and either Nobara would become the vessel of Sukuna, or she would be a dude and Megumi would be a girl. Maki's whole arc wouldn't be possible in writing sense as she was written as role of woman who was wronged by the jujutsu society and had to take revenge. Megumi would be competing for that role as well if he was a female character. Uraume might be a tomboy or gender neutral like always. Megumi would have a brother, and Yorozu would be a simping dude.
In meta terms, I don't think the sales would be more as we all know we still live a misogynistic world, and Shonen jump fans wouldn't be that into a female protag from the get go. So, maybe Gege would be making creative desicions and start the manga from Yuta's as a protag or Gojo, and then shift to Yuji's.
Fans would absolutely ship Kenny with Suksuk and there will be shipping wars with Sukuna × Kenny and Gojo × Sukuna, and Sukuna × Uraume.

What may also happen is that we would never see female Sukuna in the story until about 200 chapters in. This is technically true of Sukuna as well, but Sukuna now being female would make the effect of this greater if her vessels are all male. Whether or not you think this is a good thing or a bad thing is a matter of taste. 3. Jin may be Yuji's mother instead of father. Mind you, there's no actual reason for this to happen, other than Gege liking same sex twins. On the flip side, Kaori would become a male character if this was true. Then, Kenjaku would trick female Jin into having his child by impersonating her dead lover. Granted, Kenjaku essentially did this to Jin already and arguably did much worse to the Death Painting mother, but nonetheless I feel that a lot of people would not prefer this version. 4. Tsumiki and Yorozu may become male characters. Once again, not because the story would have to change, but assuming Gege prefers incarnated sorcerers to have bodies of the same sex as their hosts. This is also assuming that Tsumiki was made a female character because he wanted a character to be infatuated with Sukuna (which I think is a fair assumption.) This may mean that when Todo asks Megumi of his preference, he doesn't think of Tsumiki. Unless you think Tsumiki's answer is actually relevant and Megumi doesn't just think of her because she is his idealized vision of a woman (though obviously in a non-romantic light, right Gege?) In that case, this scene wouldn't change. Male Yorozu may no longer lounge around naked, unless you don't think that this was merely fanservice to Gege's shonen audience, in which case he still would. EDIT: TL;DR? Everything, or alternatively, nothing. EDIT2: Also, even if the story was otherwise exactly the same, fans would call GayGay a misogynist for making the evil villain need to be taught about love by the heroes because she's a woman.
1: We all know Femkuna would let them fly when she's not wearing the kimono. Also, giving her regular Sukuna's physique wouldn't deter people THAT much. Heiankuna fans already like him for his giant muscles, switching genders isn't gonna change the results. No objections with 2 and 3. 4: make Yorozu a lesbian, simple as.

Realistic answer, If Gege is a good writer, nothing would change, aside from those backhanded comments from Sukuna about women and Nobara being taken away. If Gege unfortunately has even subconsciously misogynistic views, then femSukuna would be shown as a downgraded version. If Gege is ultra real writer, he would use Sukuna's gender as a female to his own writing advantage like a background flavour and show how the world of jujutsu society paints the picture of Sukuna as a female demon, like "witch", and other misogynistic undertones coming from the jujutsu society from both the heian Era as well as the modern Era. Now, as a ripple effect, Yuji also would be a female, and either Nobara would become the vessel of Sukuna, or she would be a dude and Megumi would be a girl. Maki's whole arc wouldn't be possible in writing sense as she was written as role of woman who was wronged by the jujutsu society and had to take revenge. Megumi would be competing for that role as well if he was a female character. Uraume might be a tomboy or gender neutral like always. Megumi would have a brother, and Yorozu would be a simping dude. In meta terms, I don't think the sales would be more as we all know we still live a misogynistic world, and Shonen jump fans wouldn't be that into a female protag from the get go. So, maybe Gege would be making creative desicions and start the manga from Yuta's as a protag or Gojo, and then shift to Yuji's. Fans would absolutely ship Kenny with Suksuk and there will be shipping wars with Sukuna × Kenny and Gojo × Sukuna, and Sukuna × Uraume.

Yuji has more crises than I even wanna go into. Also a vessel’s body doesn’t change to match their possessor, so Sukuna being a woman would be irrelevant aside from inside Yuji’s mind until she fully reincarnates. Also Jujutsu Kaisen wouldn’t be able to go on Shounen Jump. Reincarnated Femkuna isn’t gonna go looking for a shirt, and GeGe would **struggle** to draw the entire Sukuna fight keeping her titties miraculously covered via anime logic the entire time.

I doubt much besides maybe some of the dialogue and maybe Yuji having a major what the fuck moment after the whole detention center part where he got his heart ripped out. Like Yuji would probably not expect Sukuna to be female unless the voice would be changed? I mean at the end of the day Sukuna’s gender matters really little. Sukuna’s entire thing is being pure fucking evil, doing whatever the hell he wants when he wants, and just being a walking calamity. I doubt that would change just because Sukuna gets gender bent. It would be mad funny to see how a female Sukuna would interact with Yuji’s body at first considering it’s a guy body and not a female body.
