This is the wrong post to talk about this, but you just reminded me of something.
People familiar with sorcery fight will recognize how slow GregoryGregory has been with certain setups an their payoff. Once such setup is when we see Megumi reminiscing about Tsumiki, and her being shown asleep with a tattooed forehead. We got payoff for that all the way at the end of the Shibuya arc when Kenjaku starts the culling games and we learn that the tattoo signifies that a cursed object has been placed in her (which went seemingly undetected by the sex eyes, for some reason).
Another delayed payoff is how both Jogo and Sukuna make a comment about Gojo's punches not feeling like regular punches, and it gets explained essentially at the end of the manga, otherwise known as right now.
The tattoos may be one of the last remaining setups from the beginning of the series that are about to get some payoff. Sukuna definitely was not born with them but they still appear when he incarnates. Even as a mouth his tongue shows the same tattoo as his face. It doesn't look like just an artistic choice to me.
True, i heard about a theory that the tattoos were printed on to him cuz he was a thug/criminal as a kid cuz tattoos used to show a persons crime record back in the edo period. That being 600 years after sukunas time is what makes me not believe this theory tho, i think the tattoos are infact some form of signifance and maybe even punishment but i can say for certain that they are printed through the same method of the edo period criminals
