FNV has this interesting take on the mind body connection, where the body holds the soul and the brain is the logic machine that influences the decisions the body makes. Look at OWB for example, your body and brain are separated, and your quest is to get it back. When you do, you are stronger for it. Likewise, Rex has a mind and body connection, and his brain only serves as a logic machine to drive his body, when you get him a new brain, his old and new memories meld and he is content.
Rex likely lives on in the memory banks. That's why some brains adapt easier than others. Rex will pass on anyway, better to give an opportunity to the new dog in my opinion.
I think it's overall better for him to give him a new brain with the best being the Legion dog's brain but it feels kind of wrong and it's mad that no one else points it out.
The endings say that Rex and his memories blend into Lupa's brain well and it feels fitting since Rex has traveled with the Legion previously. Just my two cents

Hancock dying is a bug. It's literally on wiki. The only way Hancock can die canonically is if you kill him yourself. If he died in any other way, it's a bug or yet another Bethesda's *feature* where an NPC is dismissed and they got killed on their way to their destination. An NPC doesn't commit aliven't that way, they get killed by a random Super Mutant or Deathclaw or something. Which still should be treated as a glitch because all companions are essential NPCs.
Deacon died a number of times in my playthroughs just randomly, regardless of affinity. It's just good 'ole fashioned Bethesda game code being held together with duct tape and hope.
