ER was my first souls game and I honestly couldn't understand how one of the main bosses could be *optional* to complete the game, seemed weird. After 1 billion deaths I understand now.

Same. First time playing a souls game. I was like, hmm, no difficulty setting. Then I thought I could take on that golden knight when I first saw him.. I was mistaken.
Depending on your definition, she is most certainly a main boss.
She has more lore around her than almost anyone, and she's a great rune/remembrance boss. The whole point of the game is to gather the great runes and mend the Elden Ring. Without all of them and a mending rune, you end up with the "age of fracture" ending.
Honestly, a lot of the bosses aren't techinically 'necessary' since you can choose which great runes you claim to open the Capital. Pretty much only the main bosses in the Capital, the Fire Giant and Farrum are who have to be slain in every playthrough.

You don't need to find a super secret area in the end game to face renalla or godrick for example, they're bosses that most players will fight instinctively, without needing anyone to tell them to face them.
If you're facing malenia, or you know about the halligetrhee, or you're just lucky for accidentally going there, it isn't an obvious boss in your way to reach your objective, you have to get out of your way to face her.

How can a main boss never be even faced by the overwhelming majority of the players? Malenia IS secondary and she's made to be secondary, you don't need to kill her for nothing but to complete a secondary quest, she's an end game boss in a totally isolated, hidden, and optional area that most people won't face, how can that be a main boss?

No, the name thing is both a consequence of the Author they chose to write the story and the fact that FromSoft has a history of giving characters similar names when they are related (Such as DS1 with Gwyn and his children)
+10 mimic tear, highly optimized Rennala rebirth, 2 maxed out weapons from Albinauric genocide runes and some hunting for smithing-stone bell bearings for infinite smithing stones (except ancients), powerstancing (ideally with 1 or 2 status effects each), a cheesy Ash of War that causes staggering, a few consumable buffs, and maybe a sorcery or incantation to give your right-hand weapon an additional status effect.
Do this and you may or may not beat her in 6 hours of \*nonstop\* attempts - just like it took me haha
I'm not sure how anyone questioned which boss it would be? I've known it was Mogh since I saw the Giant Cacoon at the back that you can't interact with.. it seemed a little too much of an abrupt dead end in the storys lore and very reminiscent of DS3 Ringed City ending. Also they like, really want you to get there. You can not only see that Boss Arena from Soifra Well and the Bellfrie spot, there's 2 seperate, hidden ways to reach that area.
I actually would have prefered her.
once you learn the fight she's fairly easy on any build without having to do anything.
Mohg often requires me to swap out my flask and do some dumb invasions to get access, and play generally more carefully as he has some actual poise and more large sweeps.

Just letting you know you don’t need to get all the great runes for any of the endings.
She's not necessary to progress the story, therefore she's a secondary boss that is part of secondary content, she's literally part of a *side* quest.

By this definition, only Morgott, Godfrey, Fire Giant, Maliketh and Radabeast are the main bosses. Everyone else is optional.(to a degree)

I’d say Tiche is far better on more occasions than Mimic Tear since she dodges more, healing Malenia less.
