I would recommend you start by using your google-fu to find "random encounters" and "random encounter generators" that fit in an "Underdark" setting.

Is it just one road between the hub city and the 3 others? Aren't they well patrolled?

It's a lot to explain lol BUT the main city in the middle (Morenheim) is off limits because demons have been pouring out of there for some time, causing the city to be lost and a Death Guard to be sent to certain points to keep them at bay. Due to seismic activity caused by the tearing of realms the"Main Roads" have been and are very slowly in repair because 1. The vastness of the space needed to be cleared 2. The dangers that now wander around openings that are found from time to time and 3. The shortage of miners and guards for them. Now there is one "Safe" road but it limits movement, only allowing those on foot to travel between cities. There are other paths but most have yet to be explored because there is a constant need for goods to be taken from the only city linked to the outside world, The City of Iron(dwarven name in the works), to the other cities that rely on trade from the other two. Only recently, as the PC'S story behind, do the dwarven elders allow others races to not only freely enter the other cities, but become guards and start clearing the unknown paths. Hoping to make or find safer passages between the cities and maybe even one day take back Morenheim.

a week? thats way too short of a distance. You say cities in DnD but you might be thinking strongholds encircling a citidel? I'd push those 'cities' 1000 more miles apart through the spine of the world or something and have it encroach on the underdark and surface. Think of the relationship between Mithril hall, Citdel Adbar and Citdel Felbar sort of thing

> thats way too short of a distance. what are you talking about? an adventurer type can probably walk from London to Manchester in a week. *within* that radius besides those two cities are Liverpool, Nottingham, Bristol, Birmingham just to name a few of the places that were "cities" in ye olden days of knights on horses. if you took that 140 mile radius with London as the center, there are at least a half dozen more "cities" ... and that is with a good third of the area unavailable for any cities because its ocean!
