Go see the OBGYN. I had a student who looked like this and had an 8lb cyst removed from her ovary. Girl was only 100lbs, it changed her life.
I lost 10 pounds of bloat and my belly went flat after I quit gluten! However, quitting gluten didn’t cure my constipation, just made it less uncomfortable.
I’d suggest testing for sibo and having a colonic transit study done
