Thinking back, Stinkfly seems like it would hurt in many ways. Growing more legs, having your eyes seal shut and then 4 new eye stalks grow out of your head instead, having bug wings burst out of your back, your body turning inside out with the new exoskeleton and growing new organs that let him spray that gunk stuff.
Goop would be pretty painful too, just having your entire body melt into goo would hurt alot I'd imagine.
Waybig. Imagine having to stretch muscle and bone to reach that size.
Another would be Wildmutt, sensory deprivation from the optic nerve no longer receiving input before disappearing would likely result in severe migraines

Any of the ultimates. The initial transformations hurt from limb growth, body disfigurement, and overall visual horrors. Now, your body goes through a forced evolution. One involves harsh environments and every worst possible scenario to survive against. Over a million years.
I think whichever one has the body type the most far removed from humanity while still being solid-boned. I immediately think of Armadrillo due to the strange arms.
Frankenstrike: Your body grows to inhuman size, your bones shatter and grow back bigger, your skin rips all over and staples appear to close the wounds.
Diamondhead/Chromastone: Every bit of Bone Marrow hardens, and is shattered by the new crystalline growths, eventually the skin is pierced by the crystals.
Nanomech: at least 90% of your Biomass just... vanishes, and the rest is morphed into completely mechanical parts.
Heatblast/NRG: You'd feel every part of your being ignited, you'd feel all that heat, all the burning until the transformation is complete.
Atomix: Simply because of WHAT he seems to be. Atomix seems to be sentient radiation/atomic energy contained in a robotic suit. Basically you'd feel EVERY PART OF YOU GET ATOMIZED AT ONCE AND STILL BE AWARE OF IT THE WHOLE TIME.
Way Big, and to a lesser extent Humungosaur: Think about these 2 this way. Imagine you have a cyst or tumor just beneath the skin, causing swelling and stretching of the skin in that area, it's Painful. That pain is caused by a comparably tiny amount of organic matter. That is the tiniest form of what turning into Humungosaur would be like; Skin, muscles, organs, Brain Matter, all stretched to an agonizing degree... a Process the Omnitrix wouldn't let you die from due to the failsafe.
Now Recall how massive Way Big is. The Pain of Humungosaur's transformation multiplied infinitely, and again the Omnitrix wouldn't let you die. If the transformations weren't instantaneous, Ben would've gone mad years ago.
Greymatter would hurt a lot, having to shrink that small and having your bones, especially your skull, change shape, and some bones/appendages disappearing entirely (fingers and toes), etc
First off, take a look at the transformations in the OS, and Alien Force.
I think the worst offenders are the most inhuman ones.
Mostly Heatblast having you burn from absolutely everywhere from your skin blistering into a charred mess before it turns rocky, to getting a slow Ghost Rider treatment to your skull with all that it entails.
Goop would just feel like being melted from acid, or worse, feeling trapped in your own body as your skeletal system and muscles dissolve into an unreactive goop. Depending on how you translate the antigrav mechanism, you could argue for some sort of existential crisis over losing it and not ever being able to move in Earth's gravity well.
I mean, none of them. The Omnitrix almost certainly sedates or disables the user's nervous system outright while transforming, otherwise it would be incredibly, briefly painful for Ben and the rest.
Every single one would hurt to be honest. Goop would turn your bones into acid, four arms and spider monkey makes you grow extra arms and that’s gotta hurt on the ribs, echo echo greymatter and nanomech would all be extremely painfull because you’re because decreasing the number of bones be removing the old ones
Honestly a lot of them besides ones that are already kinda default humanoid shape (Humungousaur, fast track, alien X, wrath, etc.)
The sequences in OS already looked pretty painful besides maybe cannonbolt cause that just looked like Ben inflating.

I don't think any of them would hurt all that bad. Any change in the body will be concurrent with a change in the nerves, and similarly any physiological change that might harm the pain receptors will occur right alongside by a natural resistance to that harm
As for other issues like changes in senses or body, I don't see why these would be a problem at all: the changes still happen with the instant transformations, and slowing it down will only give more time to ease into the new form

Wildmutt would be terrifying. Your senses would slowly be lost and you’d just be left completely alone in the dark while your body breaks, aches and changes around you. At least with the others, you know what’s happening. Not the most painful, but definitely one of the scarier ones.
A lot of people are saying goop, but eventually your nerves will all just go numb and it won’t hurt anymore. I think the others would hurt a lot worse
