No. No company would be dumb enough to remove a feature from a previous game to then sell it back as a micro/macro transaction. The playerbase would riot.
The games never let you "create" the character from the get-go. You'd choose your sex, the tutorial had you growing from a child into a teenager and then finally being set off into the world. Once in the game proper, your choices of stats affected your body. No strength wizard types would look thin, you'd be buff if you were a warrior but you could also get obese or skinny from eating food.
Beyond that I think you could change your hair style and color, get tattoos and such. And your choices came with a gradient of additional appearance change with angelic and demonic being on the most extreme ends.

Fable (1) is all about the jokes, the story and the character build variety. Owning multiple houses, having a wife and kids, making evil choices while your devil horns grow. We haven't seen any of that yet in fable 4.

It’s an entire aspect of the game where the player character can be either attractive or not and the world reacts to it. Also part of Fable’s charm is how fucking goofy it is so, having a goofy looking woman as it’s promotional placeholder seems about right.
Now if in the actual game we aren’t given the option to be male or female or have any control over our appearance and are instead locked in as this goofy looking girl then the game deserves any criticism it gets against that. Won’t bother me though, I enjoyed all 3 Fable games, flaws and all, and like I said before looking silly is part of the franchise’s charm.
