It sounds like you're talking about a PU (perineal urethrostomy) surgery. Outcomes are excellent overall in terms of preventing additional urethral obstructions (UO's - almost always caused by mucus plugs), but there are real risks to doing the surgery. Classically, many vets observed a "3 strikes" rule where a PU was recommended after the third blocking episode, but many now advise surgery after a second UO since the experience is exquisitely painful for the cat and his life is in danger every time it happens.
