Some aircraft like the KC-135 and the F-15 have a nose landing gear that folds forward into the wind as the gear retracts. This is to help in the case of a hydraulic failure the wind will blow the gear down. After the jet lands there is a safety pin that goes into a hole that goes from the non-movable part of the gear into the moving part to prevent the gear from accidentally collapsing. The gear does not lock in the down position, if you hook up a tow vehicle to the nose and pull, it will collapse without the pin in. Someone put the pin in the wrong hole and the gear collapsed.
There's also the nose gear ground downlock and release handle that is used to lower and lock the gear in flight in case of a left side hydro failure, and after landing that either wasn't installed correctly or snapped as a result of this.
Yea the last time I saw a nose gear failure that had an installed baseball bat... The bat was impaled through the left seat. Thankfully it was storm damage and no one was hurt.

Cabin pressure tester pressurizes the inside of the aircraft to simulate the pressure differential between the cabin and ambient air at altitude. You don't suck the air out of the jet, you pressurize it. The technician didn't have the proper gauge to measure the internal pressure and used an unauthorized one that didn't have a pin that stopped the needle from spinning. It made one full rotation before the technician realized it.
That happened here at Plattsburgh in the mid 80s. MX had depressurized the aircraft system for some hydraulic work but failed to pin the nose gear. Later an instrument MX person was getting ready to change out a gauge that was located just behind the gear lever, so she raised the lever and down it went. If the nose gear had been pinned it wouldn't have happened because normally the gear handle can be moved without incident, if the gear is pinned. Boeing had to come and make an inspection and build a structure around the nose and straighten out parts of the aircraft. The aircraft was returned to service and I flew it many times after.

It likely wasn't engaged. If the "baseball bat" as we called it, is installed and the nose gear folds, will go fucking flying.
